Application form

Eligibility criteria:

You have 1 to 4 children aged 0 to 17 OR you are more than 20 weeks pregnant.
You are a citizen or permanent resident, and you live in Quebec.
You live or have lived in the Montréal Metropolitan Community (CMM) for at least 12 months over the past two years.
The total gross income of the people listed on the application for the previous year is equal to or less than the following amounts:
- Single person: $44,000
- Couple: $44,000
- 2 people (not living as a couple) or 3 people: $47,000
- 4 or 5 people: $65,500
- 6 or 7 people: $82,000
- 8 or 9 people: $89,000
- 10 or 11 people: $92,500
- 12 people or more: $95,000
You are not a former HLM tenant whose lease was terminated due to eviction, non-payment of a debt to the landlord, or following a judgment by the Régie du logement.

Proof of housing for the past 12 months (lease OR a letter confirming cohabitation (parents, friends), including the housing cost and duration, signed by the person).
Proof of income (notice of assessment from the last year, any proof of current income if needed)
Legal custody (birth certificate(s) of the child(ren), proof of the legal judgment (if applicable)
If you are an immigrant: photocopy of your permanent resident card or a photocopy of your immigration document (IMM 5292), formerly (IMM 1000), or your Canadian citizenship card if you were born outside Canada.
